BY FAX...
You may use this method as an alternative to emailing your credit card payment. Fax the completed form to (973) 571-0601.
BY MAIL...
Print out the completed application and mail it with your check or money order to the address shown at the top of the form. You may also use this method for submitting payment by credit card.
